Health and Wellness ServicesĀ at School

Our Health Services teams are trained and certified to support students while at school. Services include:

  • Cumulative medical record management
  • Review and interpretation of student medical and special needs
  • Vision and hearing screening
  • Emergency care for illness or injury
  • Medication administration (prescription and over-the-counter) as authorized

Visiting the Nurses' Office

Passes:

  • Students visiting the Nurses’ Office during class must get a pass from their teacher.
  • The following passes can be obtained from the Nurses’ Office with the appropriate medical documentation:
    • Elevator pass
    • Hall pass to leave class early
    • Health pass to access the nurses’ office without a teacher pass
    • Physical education accommodation

Health-related absences:

  • Medical provider note is required to return to school if a student absence is due to:
    • Illness, including communicable disease
    • Surgery
    • Hospitalization
    • Homebound instruction
    • Notes should include any physical activity limitations or rest recommendations.
  • Students with casts, crutches, or wheelchairs must provide documentation from a medical provider outlining limitations.
  • Students returning to school after psychiatric hospitalization must provide a note from a psychiatrist, clinical psychologist, or clinical social worker.

Community Healthcare Services

Students who do not have a primary healthcare provider may receive vaccinations and certain health services through community resources.
 
If your student is uninsured, underinsured, or has Medicaid and does not have a primary healthcare provider, please contact your school's Nurses' Office to learn more about host clinics offered throughout the school year to support students’ health needs.

Action Plans and Medication Authorization

Medication (prescription and over-the-counter) that will be administered during the school day  requires a Medication Authorization Form.

  • A parent/guardian and licensed physician, nurse practitioner or physician assistant must complete the form to allow medication on school grounds.
  • Parents/guardians should make an appointment with the Nurses’ Office to drop off medication.

Students may only self-carry Epinephrine (allergy) or an Asthma inhaler.  All other medication (including over the counter) must be kept in the Nurses’ Office.

Immunization Data

D219 Immunization Data is provided to the Illinois State Board of Education each year.
