The Health Center staff includes nurses and assistants who provide care, manage student medical records and support students' health and wellness while at school.
Health Requirements
Illinois state law requires certain immunizations for students enrolled in school. The Illinois Department of Public Health provides a list of Required Immunizations by age and grade level.
- Immunization dates and records must be signed, stamped, and dated by a healthcare provider and submitted to your school.
- Immunization dates must include month, day, and year and meet the minimum dose requirements.
- Students who do not have current immunization records on file by October 15 will be excluded from school until requirements are met. See grade-level requirements for details.

Exemptions
If you are requesting a medical or religious exemption from any of the Illinois state-required vaccinations and immunizations, please submit forms with the Certificate Child Health Examination Form to avoid school exclusion.
